Instructions for your Clinical Case Study assignment
The purpose of this assignment is for you to demonstrate your understanding of the concepts and issues presented in this course. Your case study should be based upon yourself or a fictional patient, and may be factual, fictional, or some mix of the two. You are NOT required to disclose any personal or sensitive information.
A case analysis consists of several components:
background information
demographics
psycho-social factors
medical factors
education
family
etc
assessment
instruments, if any
diagnosis, using the DSM-5 classification system, be sure to add the F code also
etiology (cause of disorder)
predisposing factors (environment, family, genetics)
precipitating factors (Lack of coping skills, use of drugs, no support group)
dynamics
presenting symptoms
course
treatment recommendations and your approach
prognosis (outcome of treatment
Your assignment is in 2 phases:
Part 1: Provide an initial and response post in the discussion board about your patient. It is to be professionally written and substantive. It will be thoroughly graded, so no use of slang terms, spelling and grammar will be reviewed. Your initial post should contain the following information: name of patient, what brought them to counseling, your clinical impression and your treatment approach. Remember, to earn full credit, an initial post and a response to a classmate is required.
Part 2: Complete a detailed case study of your patient including all information as listed above. Be thorough with your details and include all pertinent information as we have discussed in class. Use your textbook and DSM 5 as your guide. Make sure to add citations to support your information and remember, it will be submitted for plagiarism, so if anyone wants to try, I dare you!!!!
