CAHPS® surveys ask patients to report on their experiences with a range of healthcare services at multiple levels of the delivery system. Several surveys ask about experiences with ambulatory care providers such as health plans, physicians’ offices, and behavioral health plans. Some of the surveys ask about experiences with care delivered in facilities such as hospitals, dialysis centers, and nursing homes.In this project, you will apply concepts learned in Assignment 3: Measurement Resources. You will use one of the four surveys you reviewed in that assignment as a guide to create your own survey as part of this project.As a reminder, the four categories of CAHPS surveysLinks to an external site. and the types of surveys within them are:Patient Experience With Providers: CAHPS Clinician and Group Survey or Hospice; Home Health Care; Surgical Care or American IndianPatient Experience With Condition-Specific Care: Cancer Care; Mental Health CareEnrollee Experience With Health Plans and Related Programs: Health Plan; Dental Plan; Home and Community Based ServicesPatient Experience With Facility-Based Care: Emergency Department; Hospital (Adult or Child);In Center Hemodialysis or Nursing Home; Outpatient and Ambulatory Surgery.Final Project InstructionsStep 1: Review an Existing Validated SurveyReview the four surveys that you analyzed in Assignment 3.Select one of the four surveys that interest you and would be applicable to current or future employment.From that one survey you selected, select five (5) closed-ended survey questions. Cut and paste into a Word document.Convert the five (5) closed-ended questions to five (5) open-ended questions (but save the original five closed-ended questions. Note: Change to an open-ended question by eliminating the answer choices and leaving a blank for the person to fill in an answer.Now you should have ten (10) questions, five (5) open-ended and five (5) closed-ended questions in your Word document.Step 2: Create a Survey in SurveyMonkeySet up a free account on SurveyMonkey (if you already have an account, you may use it).Select “sign up” in the upper right-hand corner.It should show you a registration form for a free account.Note: Remember your login name and password because you will have to log back in to get your survey results.Use the ten (10) questions you created above (five open-ended and five closed-ended questions) to develop a survey in SurveyMonkey.Post the survey in the module’s SurveyMonkey Links Discussion area by the middle of the module week. Confirm the topic by indicating the assigned topic in your thread.Step 3: Answer Posted Student SurveysRespond to five classmates’ surveys using the links posted in the Survey Assignment Discussion Board by the due date in the Calendar of Activities.Indicate in the reply that you have taken the survey.Note: The answers to the survey will post automatically to your SurveyMonkey account.Step 4: Evaluate Your Created SurveyYou will use SurveyMonkey to evaluate the different responses between open- and closed-ended questions once five or more students have answered your survey. Remember, five of the questions were close-ended questions, and five were open-ended questions.
