Select a scenario where an accident occurred, and human error played a role in contributing to the accident. You can use any example that you may have experienced at your workplace or researched on the internet. Some ideas of examples include a car accident, train accident, air crash, boating/ship accident, fire in a manufacturing plant, medical error in hospital setting etc.

As a human factor’s specialist on the investigation team, you are asked to provide a report analysing the role that human factors played in this accident and propose solutions to prevent the accident from happening again. You should answer the following questions in your report:

(a)Discuss your chosen scenario by recounting the events that occurred leading up to the accident. Provide a timeline of the major events that took place leading up to the accident.

(b) Choose two human factors concepts that you’ve learnt about in this course. Discuss the concept in detail and illustrate using examples from your chosen accident how human limitations in cognition could have led to the accident you’ve chosen.

(c) Examine the role of human error in the accident using the HFACS method. Present your analysis of the latent and active failures using the HFACS framework.

(d)Propose what changes can be made to reduce human error and prevent a similar accident from happening again in the future. Your solutions should be creative and clearly described.
