Based on previous assignments and your reading, write a case study using the EBP Model Case Study Worksheet to illustrate the processes and outcomes of an evidence-based practice approach. This case study can be based on experiences with an actual client, a composite of multiple clients, or one of the cases described in textbooks or other sources.

The worksheet outlines the following areas to complete.  Please note that three items below (3, 6, and 8) use the term justification. This should be interpreted to mean that scientific studies should be provided that show your selected approach was warranted given the client’s problems.

1. Brief case summary of the client (about 3 paragraphs)
2. Description of agency setting, types of services provided, and referral source.
3. Justification and description of selected assessment strategies / measures / approaches (and their results).
4. Description of engagement strategies employed.
5. Description of factors that will promote and inhibit change.
6. Justification and description of selected baseline measures.
7. Description of treatment goals.
8. Justification and description of selected interventions.
9. Description of intervention evaluation and goal achievement.
10. Description of termination process.
11. APA format; organization; clear and coherent writing.
